Handoffs & Sign-out
Contemporary healthcare delivery is fraught with frequent transitions of care; each transition is a potential area for errors to occur. To minimize the risk of errors, teams use structured tools to facilitate smooth transitions of care.
The Tulane IM Residency uses an internal handoff software called Phaedrus. You will receive access and login instructions during the first week of the clerkship.
Students are expected to enter in basic information on the patients you are directly following to facilitate the handoff to the night-float team that occurs during the sign-out process (usually around 5PM). Ensuring good handoffs is an emphasized skill for residents Links to an external site..
